Powers of Attorney and Other Estate Planning Documents

In most cases, simply completing your Will isn't enough to protect you and your family. There are several other important documents you'll need to have a complete Estate Plan.
- Medical Power of Attorney - allows you, the principal, to designate a trusted family member or friend to make medical decisions for you in the event you become unconscious or mentally incapable of making those decisions for yourself.
- Statutory Durable Power of Attorney - authorizes another person to engage in specified business, financial and legal transactions on his or her behalf in the event that her or she becomes disabled or incapacitated.
- Directive to Physicians - (also known as a Living Will) allows you to instruct your physicians not to use artificial methods to extend your life in the event you are diagnosed with a terminal or irreversible condition.
- Appointment of Agent to Control Disposition of Remains - allows you to specify one or more individuals to be in control of your funeral and burial arrangements. This document also allows you to specify your wishes regarding such arrangements.
- HIPAA Authorization - authorizes your health care providers to disclose your health care information to the people you have listed on your HIPAA Authorization form.
